Under Eye Filler Treatment for Eye Hollows and Dark Circles
Ask many people what they find most unattractive about their faces, and they won’t mention their noses, upper lips or even smile lines. It’s the hollows under their eyes. Also called “tear troughs”, indentations under the eyes give the face a sunken look, and make individuals look old and tired.

Treatment for Under Eye Hollows and Tear Troughs
The tear trough (or nasojugal groove), extends from the inner corner of your eye to the outer corner. Deep tear troughs may cause shadows that resemble dark circles. If you have volume loss under the eyes, blood vessels may appear to be closer to the surface of the skin, giving the area a darker appearance. Deep tear troughs also create a dividing line between the cheek and lower eyelid, making your eyes look sunken and aging your face.
The skin under your eyes is some of the thinnest on your body, so care must be taken in selecting the most appropriate hyaluronic acid filler solution for your needs and injecting it skillfully. Some fillers are softer than others and can be administered near the skin surface, while others must be delivered closer to the bone. The softer the solution, the less likely you are to feel it under your skin immediately after treatment. Certain hyaluronic acid solutions also absorb less water than others and cause less swelling. These solutions also have a thinner consistency, which allows the aesthetician to place them closer to the surface skin with fewer injections.
Results of under eye treatment with HA fillers are temporary and you’ll need maintenance injections every six months to twelve months. Some filler brands last slightly longer than others.
Fillers can also be used to treat fine lines, wrinkles and nasiolabial folds (smile lines).
Precautions and Side Effects
The most common side effects of dermal fillers for under eye treatments include:
- Swelling
- Mild Irritation
- Redness
- Bruising
- Tenderness at injection site
- Itching
Lumps and bluish skin discolorations may occur with some injectable formulas. If you do experience lumps under your eyes after treatment, our aesthetician can dissolve them using hyaluronidase. (Only five per cent of eye filler patients report more than minor side effects.)
Avoid green tea, fish oil, aspirin, ibuprofen and any supplement or medication that may promote bleeding for a week after treatment. If side effects last more than seven days, contact your doctor.
Injectable products for tear troughs shouldn’t be used to treat pregnant or breastfeeding women, or patients with multiple severe allergies.
Are You a Good Candidate for Under Eye Filler?
Non-invasive under eye treatments eliminate the need for risky surgery to reposition fat in the lower eyelid. You may be a good candidate for natural injectable fillers under the eyes if you have the following problems:
- Dark circles with additional shadows from under eye volume loss
- A hollow look under eyes or tear trough area or “deep-set eyes”
- Good skin elasticity and thickness
- A well-placed inferior orbital rim (bones above the cheekbones and under the eye)
- Fat pockets in lower eyelids
- Any under eye depression due to aging, rapid weight loss, or genetics

Under Eye Filler Treatment
You can sit or recline during under eye treatment, which usually lasts around 10-20 minutes. Our staff puts a numbing cream under your eyes, then marks the exact spots to be filled.
You won’t feel much discomfort during treatment. Most patients only need one syringe, but you may need two or more syringes if you have deep tear troughs. After treatment, avoid exercising for 24 hours. You can return to work and other everyday activities immediately after your appointment.

Aftercare

Bruising under the eyes will subside faster if you use a cold and warm compress before going to bed, and sleep with extras pillows to elevate your head.
Schedule a follow-up visit for two weeks after your treatment. All swelling, bruising and side effects should be gone by then, and you’ll have a brighter, more youthful under eye area. The division between your under eyes and cheeks will be smoother and less noticeable.

Maintenance Treatments
When you notice your under eye area becoming hollow or developing dark circles again, visit our offices for a maintenance treatment. Most patients need a dermal filler injection once or twice a year to keep their tear troughs looking smooth and youthful.
Between treatments, be sure to get enough sleep, keep allergies under control, and moisturize the skin under your eyes. You can wear eyeliner with confidence, and stop worrying about covering up dark circles with concealer. You can look five to ten years younger naturally, usually with just one treatment.
