No Obligation Consultation

First time patient consultation fee is $150.00.  A payment is required at time of booking your appointment. During your consultation you will meet with our highly trained, licensed medical provider such as MD or PA. They will learn about your concerns and create a personalized medical treatment plan for you.  As a courtesy,  after your consultation should you choose to receive same day treatment, the $150.00 consultation fee will be deducted from the cost of your treatment that day.

Appointment Cancellation

Your appointments are very important to My Botox LA Med Spa. They are reserved especially for you, we understand that sometimes schedules adjustments are necessary; therefore, we respectfully request at least a 24 hours notice for cancellations. The $150 consultation fee is Non Refundable for patients who are no shows, or cancel less than 24 hours before their appointment time. Patients who cancel at least 24 hours prior to their appointment receive 100% of their money back.  Cancellations can be completed via email, text or phone.

COVID-19 Compliance:

Our new normal has changed.  We must unite together, and help one another to preserve the health of all of us!  We trust that you will appreciate some changes in our office that we have made to assure you are in a safe, compliant and healthy environment.  Please note below changes we have made for this reason:

• All patients will have their temperatures checked and be questioned regarding COVID-19.
  
  
• In accordance with government regulations, we are trying to still save PPE (Personal Protective Equipment) for hospitals and COVID-19 hot spots, and so while we cannot provide masks, we require that you please bring in your own masks.
  
  
• Please, no children or companions in the office. Please come alone for their safety, safety of other patients and our staff.
  
  
• We will continue practicing social distancing by spacing appointments.  Accordingly, we ask that all patients arrive fifteen minutes earlier than your scheduled appointment time. Also, please let us know if you will be delayed in arriving to your appointment or need to reschedule, as we are allowing a restricted number of patients in the office at one time.
  
  
• While My Botox LA Med Spa has always maintained a clean and disinfectant environment by implementing sanitary measures, due to COVID-19 all exam rooms in our medical spa will be cleaned, disinfected and sanitized between each patient and after the final patient of the day. Disposable germicidal wipes will be used on any surface which comes into contact with a patient, such as bed, equipment, and counters. In the absence of appropriate disposable surface wipes alternative disinfecting substances that meet the EPA criteria may be used.
